Per Diem Hospice / Home Health Nurse Practitioner

CHANS is seeking an experiencedNurse Practitionerto join our Home Health/Hospice team in a per diem capacity. The goal of palliative care is to maximize quality of life and align the individual's medical care with what matters most to them. This position is a per diem role serving the greater Brunswick area.

Qualifications:

  • Advanced knowledge of nursing theory and practice, and additional specialized medical training, in order to provide expanded nursing practice at a level generally acquired through completion of a Master's Degree.
  • Current license and registration to practice as a Nurse Practitioner or Physician Assistant as required in the State of Maine.
  • Need 2 years' experience in Hospice and or Palliative Care- advanced certification in Hospice & Palliative Nursing preferred.
  • Palliative Medicine specialty certification and/or palliative practice experience equivalent/ eligible for specialty certification.
  • Knowledge of health care management and the hospital care environment.
  • Ability to work independently, effective time management and problem solving skills.
  • Interpersonal skills and communication skills to engage as a specialist with a variety of personnel (e.g., patients, employees, physicians, and other health care professionals.)
