JOB TYPE Classification: Nonexempt Supervises Positions: No JOB SUMMARY The MSW Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive, complex, and safe discharge plans. Collaborates with RN Case Manager's for guidance with medically complex cases. The MSW Case Manager is also responsible for facilitating coordination of care. The MSW Case Manager along with the other members of the healthcare team accepts responsibility, authority, and accountability for management of care. This shall be in accordance with the current policies and procedures, mission, vision, and values of UVA Health Northern VA & Culpeper. Serves as a resource for discharge planning and care coordination for psychosocially complex cases. Maintains current knowledge of available community resources and post-acute care options. Communication: Communicates effectively with people of diverse backgrounds and all levels of authority.
Quality: Reviews processes, identifying and recommending opportunities for improvement.
Patient Service: Actively participates in identifying areas of opportunity to improve the experience for patients and guests.
The incumbent may be asked to perform additional duties as assigned. QUALIFICATIONS Education: Graduate Degree required - Master's Degree in Social Work from school accredited by the Council on Social Work Education. Experience: 2 or more years of professional social work experience or 2 or more years of healthcare experience preferred. Case Management or related experience preferred. Refer to the Life Support Training Policy for additional details. Licensure: N/A Additional Skills/Requirements Required: Weekend and holiday coverage. Additional Skills/Requirements Preferred: Knowledge/Competence of Microsoft Office products and keyboarding. PHYSICAL DEMANDS Physical Demand Code: 3A, Administration Work Function/Activity: Sedentary to Light Physical Demand The job requires frequent sitting, standing, walking, and bending/stooping. Frequent repetitive body movements. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull up to 20 lbs. occasionally.
