The Community Health Project Coordinator supports community health and health equity initiatives across the Hospital and/or Health System in order to further the Health System goal of improving health equity for all Hospital communities. The coordinator works under the direction of the Manager, Community Health Improvement and in collaboration with the system community health team to facilitate project management, programs and initiatives, surveys, data collection and reporting, evaluation, and resource development. The coordinator supports the Hospital and/or Health System and the collective impact partnerships including the Community Health Needs Assessments (CHNA) and Community Health Improvement Plans (CHIP) and participates in the system CHNA and CHIP process. The coordinator works in partnership with internal stakeholders and community partners who are collaborating on community health initiatives. The coordinator is an active member of the Office of Health Equity and Community Impact community health team.

Responsibilities

  • 1. Supports the community health work of the Hospital and/or Health System and represents the Hospital and/or Health System in the Hospital and Health System regions.
    oAssists in identifying community needs.
    oHelps develop, implement and track programs and initiatives to improve the health and wellbeing of local residents in collaboration with Hospital and/or Health System leadership and departments.
    oParticipates in grant writing opportunities and grant program implementation.
    oBuilds relationships with community partners and identifies opportunities for engagement.
  • 2. Supports the Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP) process for the Hospital and/or Health System and the collective impact partnership (s).
    oTracks CHIP progress and prepares bi-annual dashboards and reporting of CHIP accomplishments for Hospitals and/or Health System and collective impact partnership.
    oOrganizes meetings and output of the Hospital Community Health internal stakeholder group.
    oAssists in development and reporting of Hospital and collective impact partnership CHNA.
    oParticipates as an active member of the system community health team in the development and implementation of the system CHNA and CHIP process.
  • 3. Coordinates the efforts of collective impact partnerships (steering committee, work groups and task forces).
    oSchedules meetings and presenters
    oPrepares agendas, takes minutes and keeps records.
    oConducts surveys, analyzes and reports on results.
    oPrepares reports and presentations.
    oManages communication including flyers, website and social media.
  • 4. Provides information and serves as a resource for Hospital and/or Health System on community health and health equity.
    oCollects, analyzes, tracks and reports on data for Hospital and/or Health System community health and health equity programs and initiatives in collaboration with Health Equity Epidemiologist to measure health improvement over time.
    oGathers information to support Certificate of Need and Hospital departments education or initiatives.
    oResearch national and state best practices and national, state and local trends.
    oAssists in annual communication plan.
    oInvolved in website development and maintenance.
    oInvolved in social media efforts.
    oDevelops and prepares surveys, reports and presentations.
    oGives presentations as needed.
  • 5. Participates as a member of the Office of Health Equity and Community Impact community health team.
    o Involved in system community health planning, programs and initiatives.
    oFunctions as active member of system community health team.
  • 6. Assists, coordinates and manages special projects as requested. Performs other duties as assigned.

Qualifications

EDUCATION

Bachelors degree with experience working in a health or social determinants of health related environment. Master's degree in public health, business, public administration, health policy, or healthcare management preferred.

EXPERIENCE

A minimum of 2 years of experience in community health, public health, or social work required. A minimum of 2 years of successful community engagement related work experience with community partnerships, community members, community-based organizations and/or governmental agencies preferred.
Demonstrated ability to work in a team-oriented, highly iterative, and collaborative environment.
Demonstrated ability to establish effective interpersonal relationships with a broad range of internal and external constituents.
Experience with healthcare research preferred.

Ability to work collaboratively with diverse community and other stakeholder groups.
Demonstrated ability to foster cooperation, communication and consensus among groups and teams as well as lead, influence, coach, and guide others to help them achieve desired results.
Possess strong computer skills including proficiency in Microsoft Office Suite.
Proficient in data analysis and creation of action plans.
Working knowledge of survey development and survey techniques.
Social media skills preferred.

PHYSICAL DEMAND

Ability to travel to sites to support community health activities.
