Job Title: Access Case Manager
Location: New Brighton, MN
Compensation: Starting at $49,920 per year

Position Summary
Beauterre Recovery Institute is seeking a dedicated Access Case Manager to join our team. This role is critical in supporting individuals navigating recovery and accessing necessary services. The Access Case Manager will conduct assessments, build relationships with clients and stakeholders, and facilitate seamless entry into care programs, ensuring a high-quality experience for all.

Key Responsibilities



  • Conduct brief biopsychosocial assessments to determine program fit and appropriate level of care.
  • Build rapport with clients and community stakeholders to ensure exceptional service delivery.
  • Use knowledge of the 12 core functions and systems theory to support person-centered care.
  • Assist clients in accessing essential services, such as financial assistance, housing, and healthcare.
  • Navigate community resources to support client needs and connections.
  • Verify program eligibility through client contact, chart reviews, and collateral records.
  • Coordinate service initiation, including planning, transportation, and barrier resolution.
  • Maintain communication with clients regarding referrals, admissions, and financial responsibilities.
  • Monitor waitlists, verify funding, and manage client financial arrangements.
  • Support transitions to care teams through warm hand-offs and accurate documentation.
  • Build and maintain relationships with referral partners to enhance community engagement.
  • Perform necessary data entry, maintain accurate records, and track the daily census log.


Qualifications
Required:



  • Bachelor's degree in Human Services or Business-related field.
  • Valid licensure in social work or counseling (e.g., LSW, LGSW, LISW, LICSW, LADC), or at least 4 years of case management experience.
  • At least 1 year of experience in a co-occurring disorder program (substance use and mental health).
  • CPR certification (or ability to obtain prior to hire).
  • Knowledge of program rules, regulations, and requirements.
  • Proficiency in computer-based documentation and communication tools.
  • Valid MN driver's license.


Preferred:



  • Three or more years of experience in a chemical dependency facility.
  • Two or more years of experience in sales, marketing, or business development.
  • Strong organizational and interpersonal skills, with attention to detail.
  • Experience adapting to changing priorities in a fast-paced environment.
