The Care Management Associate Per-Diem shall be responsible for the following:
  • Under direct supervision, the Care Management Associate provides accurate and timely non-clinical administrative support in the assigned team/unit. Coordinates all aspects of the patient referral process, collaborating with providers, patients and families, and community resources by CMA department policies and procedures.
  • Coordinates the patient referrals process for care management, including skilled and private duty nursing, infusion and enteral therapy services, durable medical equipment, and transportation throughout the care continuum.
  • Collaborates and delivers clear, effective communication with multidisciplinary team members to coordinate the care management activities within the assigned inpatient unit/team.
  • Works with insurance to obtain prior authorization approvals. Plan and Prioritize clinical scheduling and follow up on all referral requests according to permissions.
  • Serves as a liaison with the relevant clinical groups ensuring regular and fluid communication and developing connectivity between these areas throughout the care continuum. Coordinates with case management teams to confirm timely delivery of medications, equipment, and supplies to patients.
  • Assist patients and families with inquiries, provide resources, educational literature, and direct to community partners.
  • Supports continuous process improvement by identifying alternatives to existing processes and participates in pilot programs as assigned.
  • Supports the department in implementing initiatives by participating in special projects as needed.

To qualify, you must have:

  • A High School Diploma / GED is required. An Associate's Degree is preferred.
  • Two years of relevant administrative experience in a healthcare setting
  • Intermediate skills in MS Office Excel, Outlook, and Word.
