Delaware's Largest Cancer Treatment Group- Medical Oncology Hematology Consultants, Newark & Middletown, DE

We are a private practice comprised of 11 physicians and 10 APPs. Our mission is to provide quality cancer care to the patients in our local community. We are affiliated with the US Oncology Network, which is one of the nation's largest networks of integrated, community-based oncology practices that are dedicated to advancing high-quality, evidence-based cancer care. Our board-certified physicians in Hematology and Medical Oncology, Palliative Care Medicine, and breast surgery have trained and held faculty positions in nationally and internationally recognized academic centers. Our physicians have extensive experience in the diagnosis and treatment of malignant diseases and disorders of the blood and breast.

We have an immediate opening for a Clinical Pharmacist.

Scope: Oversees pharmaceutical care plans in designated specialty areas or product lines, including initial patient evaluations, development of care plans, and ensuring treatment plans are carried out. Ensures ongoing review of patient clinical status. Recommends adjustments to drug therapy based on clinical status and treatment outcomes. Coordinates with clinical providers to ensure all aspects of pharmaceutical care are delivered safely and timely. Supports and adheres to the US Oncology Compliance Program, including the Code of Ethics and Business Standards.


Responsibilities

Essential Duties and Responsibilities:

-Perform initial patient evaluations and work-ups related to disease status and anticipated prescribed therapy.

-Collect, analyze, interpret, and evaluate data relevant to patient therapy.

-Obtain histories and other patient data to optimize drug therapy and nutritional support management, maximizing comprehension of medication administration and side effects.

-Establish therapeutic objectives and monitor patients for desired therapeutic outcomes.

-Evaluate the significance of signs and symptoms relative to disease processes and recommend modifications in drug therapy.

-Detect and evaluate the clinical importance of adverse reactions and complications associated with drug therapy.

-Provide counseling and answer questions from patients, family, and healthcare professionals concerning drug therapy.

-Maintain active involvement with clinicians in the development and implementation of a standard drug order format and ordering process.

-Maintain thorough and appropriate documentation in the patient clinical chart of physician orders and communication with other healthcare professionals.

-Coordinate with physician staff and service providers in planning and implementing patient care plans to ensure safe and timely delivery of drug therapy.

-Review and perform QA checks on all new and repeat medication orders.

-May prepare or supervise the preparation and dispensing of drug therapy according to Company policies and procedures.

-Present training and in-service programs as necessary.

-Provide educational initiatives for patients, caregivers, and healthcare team members.

-Participate in policy development, advocacy, and institutional committees.

-Review and recommend treatment plans in the EMR.

-May supervise the activity of Pharmacy Technicians.

Minimum Qualifications: Bachelor's or Pharm.D. degree from an accredited school of pharmacy. Minimum two years' experience as a registered pharmacist, with an oncology background preferred. Valid state licensure as a Registered Pharmacist.
