Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. The network also includes 12 community medicine sites providing care to disenfranchised New Yorkers, comprehensive HIV services, chemical dependency programs, and a family support center that offers educational, vocational, and other social support programs. Position Summary:
We have an exciting opportunity to join our team as a Assistant, Patient Navigator (Temporary).

The Patient Navigator Assistant will be responsible for providing culturally competent anonymous and confidential trauma screening, counseling, and referrals. The Patient Navigator Assistant will provide psychoeducation related to the impact of trauma on mental health and wellness and evidenced based treatment models for trauma.

Job Responsibilities:

  • Completes trauma screeners and facilitates referral and enrollment process into the Center for Child Trauma Treatment.
  • Coordinate with representatives of other related agencies to communicate information and develop partnership opportunities for outreach.
  • Contributes and leads outreach campaigns through the development of innovative educational and health promotion resources to community programs.
  • Interacts with patients/staff in a courteous manner and communicates with other departments to facilitate patient flow and/or resolve patient issues.
  • Provides critical feedback to other members of the health care team based on observations in the field and outreach as necessary.
  • Complies with all organizational policies and procedures. Patient Navigator is expected to behave ethically, demonstrate competence in effective communication and team building, demonstrate effectiveness in working in a collaborative environment and demonstrates confidence in decision-making and in building relationships.
  • Performs other duties as assigned in accordance with the mission, goals, and values of the Family Health Centers at NYU Langone

Minimum Qualifications:
To qualify you must have a High School diploma. Experience working with a behavioral health population is required.

Preferred Qualifications:
Bilingual English/Spanish
Some college preferred. Past experience and employment working with children and adolescent populations. Experience working with LGBTQ communities. Experience with effective behavioral interventions for traumatized youth.

Qualified candidates must be able to effectively communicate with all levels of the organization.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $33,778.63 - $43,000.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
