Summary:

The Case Manager - Access to Care role is responsible for enrollment of enrollees/participants into the Access to Care program. Provides casework services including information and referral, psychosocial support and advocacy. Collaborates with healthcare, mental health, social service providers and the participant to optimize the enrollee/participant's health and ability to participate in the Program.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Bachelor's Degree preferred or equivalent relevant experience in a healthcare environment required.
  2. License/Certifications: Valid Driver's License and reliable vehicle may be required for off-site travel.
  3. Experience: Two years of experience in a community health setting required.
  4. Additional Skills/Requirements Required: N/A
  5. Additional Skills/Requirements Preferred: Ability to develop and present information to groups, ability to use database and other software systems
