At Healthcare in Action, we serve patients experiencing homelessness through an innovative "street medicine" approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems.
Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks.
The Temporary Referral Coordinator plays a crucial role in ensuring the smooth operation of various tasks related to patient data management, communication, tracking, and specialty referrals. The role involves meticulous data entry, efficient response to team inquiries, proactive tracking and monitoring, and active involvement in special projects as assigned.
**This position is remote; however, it is preferred to have familiarity with the Inland Empire region (San Bernardino or Riverside Counties) **
Responsibilities include:
- General/Data Entry:
- Create new patient charts and enter relevant information into designated platforms (Smartsheets, Canvas).
- Review and update patient eligibility information.
- Conduct audits on patient charts to ensure completeness and accuracy of enrollment forms.
- Communication & Front Office Duties:
- Effectively respond to team requests and inquiries on multiple communication platforms (Teams, TigerConnect, Email).
- Process medical record requests promptly and accurately.
- Manage incoming faxes, assigning them appropriately to patient e-charts.
- Monitor various communication platforms to ensure timely and appropriate staff responses.
- Tracking/Monitoring:
- Co-support the tracking of Cell Phone Distribution and Phone Line Reassignment.
- Team Collaboration:
- Actively participate in team meetings, contributing insights, and following through on action items.
- Processing Specialty Referrals:
- Act as a liaison between the requestor and specialty office.
- Submit all required documentation for specialty referrals.
- Identify new specialists to expand the network of specialty offices available to our patients.
- Special Projects or Tasks:
- Collaborate with the team and take on special projects or tasks as assigned, demonstrating adaptability and a proactive approach.
- The Referral Coordinator is expected to contribute actively to the achievement of organizational goals and maintain a high standard of professionalism in all responsibilities.
- Actively support the achievement of Homebase's Vision and Goals.
- Other duties as assigned.
Qualifications and experience:
G.E.D./High School, preferred, but not required.
1+ years call center or related customer service experience.
Experience in the healthcare, insurance or pharmacy industry highly desirable
Previous experience in data entry and administrative tasks.
Strong communication skills, both written and verbal.
Detail-oriented with a commitment to accuracy in patient data management.
Ability to multitask and prioritize tasks effectively.
Familiarity with medical record processes and specialty referrals is a plus.
