Description
The Children's Home of Poughkeepsie has been caring for children throughout the Hudson Valley for over 175 years! We strive to empower children and families to achieve optimal health and well being, and are looking for a CASE PLANNER such as you to help fulfill our mission. The EMPOWER Case Planner is responsible for the delivery of casework services to families and children in care. The Case Planner is responsible for coordinating services with various agencies, professionals, and providers. The Case Planner represents the Agency at criminal and family court proceedings, school meetings, conferences with LDSS, and any other entity the Agency communicates with. The Case Planner maintains accurate and detailed case records and prepares reports and other documentation required by the agency and/or LDSS.
