**Available to regular status employees who are scheduled to work a minimum of 24 hours. Southcoast Health is an Equal Opportunity Employer. Responsibilities Under the general supervision and direction of the Director, Oncology Nursing or designee, provide clinical social work services to patients and families assisting to remove barriers to care by providing interventions/referrals for financial and transportation issues. Provide case management services independently including but not limited to application of clinical diagnostic, assessment or treatment knowledge or skills. Identify and address psychosocial factors that may impede the patient*s ability to optimize benefit from the health care delivery experience. Provide age specific assessments of psychological and social functioning including determining various psychiatric diagnoses according to current DSM criteria and establishing intervention plans to address the identified issue. Screen and evaluate patients and their families for psychological distress during diagnosis and disease progression. Qualifications
