Job Overview:
We are looking for a passionate Quality Advisor who is excited about making a difference in patient safety outcomes. As the Quality Advisor you would perform all aspects of hospital wide performance improvement activities. Acts as liaison to Medical Staff Peer Review committees. As liaison to Peer Review Committees, reviews charts for Mortality and Morbidity, performs Operative and Invasive Procedure review, reviews hospital acquired conditions and patient safety indicators, and evaluates identified quality issues. Participates in hospital wide initiatives in support of regulatory and accreditation process. Abstracts clinical data for regulatory studies. Recognizes and investigates all high risk occurrences , develops plan of correction accordingly.
Qualifications:
- Licensed RN in the State of New Jersey
- Minimum of 2 years of hospital nursing experience i
- Experienced with Excel, PowerPoint and Microsoft products
- Knowledge in Epic EMR program
- Quality and Safety experience preferred
Certifications and Licensure:
- Graduate from an accredited NLN Nursing Program
- Active NJ RN licensure or Compact state licensure with ability to practice in NJ
- CPHQ preferred
Scheduling Requirements:
- Full-Time
- Day
- 37.5 hour work week Mon-Fri
- No weekends, no holidays, no on-call
Essential Functions:
- Compiles, reviews and reports out information/data to all relevant stakeholders regarding agenda items on a monthly/ quarterly basis as necessary.
- Completes chart audits/ clinical reviews for clinical outcome and performance improvement studies.
- Provides feedback to the appropriate stakeholders regarding the findings and opportunities for improvement identified through the clinical reviews. Topics included in clinical reviews/ chart audits include but are not limited to: nursing sensitive indicators, operational indicators, patient safety indicators)
- Facilitates expert assessments, reviews and interviews of high risk occurrences and provides detailed summaries
- Facilitates Apparent Cause Analysis or Root Cause Analysis of high risk events and provides detailed summaries of clinical outcomes when required.
- Maintains confidential agenda, cause/ effect timelines, interviews and minutes for all Root Cause Analysis
- In collaboration with action plan owners monitors progress of corrective action items and provides follow up until completion of action item
- Fulfills role as quality liaison to Medical Staff Peer Review Committee meetings
- Maintains confidential agenda and minutes of Medical Staff Peer Review Committee meetings
- Participates in Patient Safety Committee ad hoc meetings as indicated.
- Formulates plans for improvement and ensures completion based upon Committee recommendations/findings.
- Refers identified individual and system issues identified in Peer Review Committee meetings to appropriate person/department in the Medical Center.
- Reviews Hospital Acquired Conditions for opportunities for improvement and forwards to appropriate department for follow up
- Reviews data on a regular basis to determine compliance and identify targeted areas for improvement.
- Formulates plan for correction for indicators out of compliance.
- Reviews selected mortalities to ensure that standard of care is met and forwards to appropriate department for follow up
- Reviews all safety reports entered into the Verge system and follows up with appropriate stakeholders to review opportunities for improvement, discuss lessons learned and assist to develop action plan.
- Ensure appropriate follow up and timely closure of safety event reports in Verge occurs.
- Provides a compassionate and supportive environment with prompt and consistent service.
- Adheres to patient rights: provides for the confidential treatment of all communications and records
- Works in collaboration with the multidisciplinary team in reviewing all patients with high end devices and provides quality review.
- Leads and participates in process improvement for the program(s) and for the quality of clinical outcomes.
- Maintain database(s) for patients, outcome, and complications and submit to national registry as required
- Provide comprehensive guidance on the appropriate use of temporary devices enabling staff to deliver safe and positive patient outcomes
