Catholic Charities Maine's Behavioral Health Network is expanding! We're proud to oversee the new Freedom Place, a 24/7 residence located on State Street in Portland. This safe and supportive home serves women who have experienced chronic homelessness, substance use disorders, domestic violence, incarceration and other challenges.

We are currently seeking a Full-Time Community Resource Care Coordinator (CRCC) to join our team and help launch this impactful program.

About the Role: As a CRCC, you'll be a key member of our support team, providing direct case management and community care coordination for residents of Freedom Place. Working closely with the C.A.R.E.S. team, property management, and other partners, you'll help residents transition into the program, stabilize, and move forward in their life recovery journey.

Core Responsibilities:

  • Assist residents in understanding and integrating program and housing expectations.
  • Collaborate on Life Recovery Goal development and progress tracking.
  • Support access to healthcare, substance use recovery services, probation compliance, and life skills development.
  • Connect residents with community resources, including education, employment, and essential services.
  • Foster motivation for change, emotional stability, and healthy relationships.
  • Promote a safe, respectful, and healthy living environment.
  • Provide case documentation in line with agency, state, and program standards.

Qualifications:

Education & Certification:

  • MHRT/C (Mental Health Rehabilitation Technician/Community) certification required. (Provisional A or B accepted with a plan to obtain MHRT/C.)
  • Motivation to obtain CADC and CRMA certifications-or ability to do so within 12 months of hire.

Experience:

  • Minimum of 2 years of experience in behavioral health, case management, or working with vulnerable adults, youth, or families.
  • Strong knowledge of local community resources.
  • Proficiency with electronic health record systems and general computer skills.
  • Candidates with 2-3 years' experience with supporting individuals to access community resources will be considered.

Starting Salary: $18.00 - $24.00 per hour. Actual starting salary contingent with experience/education.
