JOB PURPOSE:

The Intake/Outreach Coordinator plays a vital role in our organization, responsible for greeting and enrolling potential clients, providing education on Endeavors services available, and collaborating with community resources to facilitate referral processes across organizations. The Intake/Outreach Coordinator will attend and host community outreach and training events to engage potential clients into services. This position is dedicated to ensuring clients can access care quickly, community members are aware of all programs and resources available to support them, and the Intake/Outreach Coordinator educates on program enrollment qualifications and enrollment procedures.


Qualifications

ESSENTIAL JOB RESPONSIBILITIES:




* Be active in and network with Hill Country community groups. Provide program overviewpresentations at various community groups within the city and surrounding area.

* Promote the success and reputation of Endeavors and its programs.

* Develop a network of partners to improve client access to local resources.

* Conduct outreach with potential referral sources; foster positive ties with third partyvendors, special groups, and community organizations throughout the city.

* Report outreach efforts weekly to the program manager and collaborate with programpersonnel to continuously improve the outreach process. Collects data on eventattendance and engagement and prepares reports as requested by direct leadership.

* Set up initial client file for clients and their families entering Endeavors programs.

* Ensure the successful completion of intakes/outreach efforts by conducting follow upattempts if paperwork or information is required to complete registration.

* Assign clients to needed assistance (counseling, wellness room, case management,medication management, etc.), and ensure completed connection.

* Prepare and keep data, files, reports, drafts and accurate reporting of intake/referral/listactivities.

* Host community mental health education events quarterly.

* Collect fiscal data to assist with the completion of monthly, quarterly and annual reports

* Assist the Program Manager in monitoring the programs referrals, intakes and wait list andreport on the programs predetermined objectives.

* Communicate with team members to ensure maximum efficiency.

* Manages daily assignments and ensures deadlines are met and work is completed correctly

* Other duties as assigned.

ESSENTIAL QUALIFICATIONS:

EDUCATION: Associates degree in Public Health, Social Work, Human Services, or related field Preferred. Experience may be considered in leu of education.

EXPERIENCE: Must demonstrate ability to foster team-building and a supportive, flexible environment.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion.

LICENSES: Driver's License with clear record.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. Spanish Speaking preferred.
