Description
JOB PURPOSE: The Case Manager will support the Texas Hill Country and surrounding communities by identifying resources and connecting individuals to services that aid in well-being improvement. The Case Manager will support clients in gathering necessary documents to register for services or apply to employment opportunities. The Case Manager will work collaboratively with local community employers to assist in connecting potential employees to job opportunities. The Case Manager is responsible for using sound professional judgment and best practices to ensure quality case management services are provided to the service population especially in crisis situations while maintaining compliance with established program policies and procedures. ESSENTIAL JOB RESPONSIBILITIES:
ESSENTIAL QUALIFICATIONS: EDUCATION: Bachelor's degree in Community Health or Social Science preferred. Experience may be considered in leu of education. EXPERIENCE: Preferred: 1+ year related experience and/or training including experience working in Crisis intervention. Proficiency in Spanish/English (written and spoken language skills), required. Preferred experience in resume writing. ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion. Employee is expected to report to work as scheduled, on time and prepared to start work. This role requires the employee to be in the office most times, and at other times, the employee will be required to remain on task in a work from home or travel environment. LICENSES: Driver's License with clear record. VEHICLE: Must have daily use of a vehicle without prior notice. OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates.
