The Salvation Army Midland Division Corps located in Columbia MO, is looking for an individual who would like to apply their experiences, to fill their Case Manager position. This individual will be responsible for the overall coordination and operation of the Columbia Social Service programs including EA and POH services. This involves, but is not limited to, program development, fiscal management and reporting for all related grant funding, and case management. Will provide intake, assessment, and services to individuals seeking EA assistance and serve as Case Manager for the Pathway of Hope (POH) program working with families and individuals referred internally and externally in Columbia.

This is a part-time 25 hours per week, $20.00 per hour.

Essential Functions

Administrative



  • Participate in scheduled staff meetings and team management meetings. Maintain effective communication with Corps Officers regarding all aspects of operations.



Case Management



  • Provide emergency case work services in Columbia to include but not limited to: rent, utilities, I.D.'s, Clothing vouchers, etc.
  • Follow the Pathway of Hope approach consisting of the following functions with a limited number of participating clients:


    • Conduct pre-screening and intake of clients using specified assessment tools.
    • Conduct goal setting steps with clients formulating a change-oriented action plan.
    • Conduct follow-up case management meetings with clients.
    • Monitor and track the changes and goal attainment on the action plan.
    • Provide persons seeking assistance with intake and assessment activities to determine client eligibility and to match clients with resources. Provide persons seeking assistance with intake and assessment activities to determine client eligibility and to match clients with resources.
    • Identify and secure housing options, programs and services for clients identified as homeless or housing insecure.
    • Provide on-site, remote, community case management to clients seeking emergency assistance.
    • Conduct community education awareness opportunity focused on homelessness prevention and homeless resources.
    • Maintain client files and complete monthly statistical reports.


  • Direct, schedule, and supervise the volunteers helping with Pathway of Hope families.
  • Communicate Pathway of Hope activities to the supervisor, coordinate social services with other Corps programming, and inform clients of Corps programming.



Program Development/Evaluation



  • Ensure that operations of the Columbia Social Services programs are in compliance with current approved policy and procedure of The Salvation Army, its National Social Services Standards, state and local licensing certification and regulatory authority. Ensure adherence to mission in all aspects of our services.
  • Develop, implement and maintain a social services policy and procedure manual reflective of all emergency services & POH operations and procedures for Columbia.
  • Develop network system with shelter to identify potential clients for POH.

Minimum Qualifications

Bachelor's degree in social services area or related field with one year of experience. Combination of training and experience will be considered. Requires excellent written and oral communication skills with an ability to work with diverse cultures. Strong leadership style and good organizational skills required. Commitment to carry out The Salvation Army's mission is essential. Knowledge of The Salvation Army and Government, State and community resources is helpful. Must be a team player, a self-starter, able to handle several projects at the same time and perform well with minimum supervision. Demonstrate ability to think independently and exercise sound judgment. Requires a valid driver's license, and pass TSA MVR check.

Physical and Work Environment Requirements

Capable of occasionally lifting 20 pounds and sometimes up to 40 pounds. Occasionally move 50 or more pounds using dolly/cart/truck. Must be capable of performing in an office environment to include using a computer, answering phones, etc. Most work will be indoors in a temperature-controlled environment. Some duties may involve leaving the main job site to attend meetings and assist clients.
