Description

This position provides direct outreach and services to individuals living on the streets, in encampments, or experiencing housing insecurity. The role is community-based and centered on meeting people in their actual living environments to deliver compassionate, person-centered care. Funded to serve patients who may not fit within the traditional medical model, this position is a part of the care team which includes medical providers, primary care teams, and community partners to improve health outcomes and reduce disparities among unsheltered and housing-insecure individuals. The role also includes advocating for patients' social and environmental needs as they arise. Collaboration with health center sites, primary care providers, local health departments, and community-based organizations is essential to provide wraparound services and build a foundation of trust that supports ongoing care for socially vulnerable populations.


Examples of Duties

Support - Direct Patient Interaction 100%
1. This position works in the streets, encampments and clinic to assess patient needs, provide direct
support, and connect individuals experiencing homelessness or housing insecurity to medical and
community resources.
2. Explain program requirements and guidelines to the general public.
3. Develop community resource materials that are up to date and reflective of the local community.
4. Conduct comprehensive interviews and intake assessments to determine the needs of individuals to be
served by the program.
5. Assist patients in setting and achieving personal goals in person, by phone or in group setting.
6. Through skill building interventions and other supportive interventions, assist patients in effectively
utilizing a wide spectrum of resources, managing symptoms of their illness, and developing daily living
skills to gain greater independence, employment and other meaningful activities.
7. Connect patients to community resources including but not limited to housing, medical care, education,
vocational training, and additional recovery resources / meetings.
8. Assist patients with transportation to medical and other related appointments.
9. Prepare and maintain written records as needed by the program.
10. Participate in all required administrative and training meetings in order to ensure adherence to policies
and protocols.
11. Complete other duties as assigned.


Qualifications

Credentials to be Verified by Placement Officer
1. Any one or combination totaling three (3) years (36 months) from the categories below:

  1. Coursework in social work or behavioral sciences, or closely related field as measured by the following conversion table or its proportional equivalent:
    • 30 semester hours equals one (1) year (12 months)
    • Associate's Degree (60 semester hours) equals eighteen months (18 months)
    • 90 semester hours equals two (2) years (24 months)
    • Bachelor's Degree (120 semester hours) equals three (3) years (36 months)
  2. Progressively more responsible work experience in social service/ counseling at a paraprofessional level under qualified supervision.

Knowledge, Skills and Abilities (KSAs)
  1. Working knowledge of human nature.
  2. Ability to interview individuals to determine their physical, social, and/or possible psychological needs.
  3. Ability to organize, prepare, and maintain records, reports, files, and correspondence.
  4. Ability to obtain services necessary to meet participants' needs.
  5. Ability to establish effective working relationships with program participants and staff.

Condition of Employment
Pursuant to the State Universities Civil Service System, an out-of-state resident who is hired into this position must establish Illinois residency within 180 calendar days of their start date.
