Opportunity:

  • Join and collaborate with experienced Family Practice Physicians to manage shared patient panel
  • Provide care of established patients for treatment and management of both acute and chronic conditions
  • Hours are Monday-Friday; No Call

Qualifications:

  • Graduate of an accredited MSN program
  • Current WI RN license
  • Current APNP
  • Current DEA number
  • CPR Certification
  • Other certifications as required

Responsibilities

Essential Duties and Responsibilities

  • Performs physical examinations and preventive health measures within prescribed guidelines and instructions of Physician. Orders, interprets, and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs. Records physical findings, and formulates plan and prognosis, based on patient's condition. Discusses case with Physician and other health professionals to prepare comprehensive patient care plan. Submits health care plan and goals of individual patients for periodic review and evaluation by Physician. Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures. Refers patients to Physician for consultation or to specialized health resources for treatment. While performing the duties of this Job, the employee is regularly required to stand, walk, sit, use hands to finger, handle, or feel, reach with hands and arms, talk, and hear. The employee is occasionally required to climb, balance, taste, and smell. This position frequently requires close vision, depth perception, and the ability to adjust focus. The position occasionally requires distance vision, color vision, and peripheral vision. The position requires the employee to exert up to 50 pounds of force occasionally and up to 25 pounds of force frequently. The position requires occasional travel and home visits.

Education and Experience

Graduate of an accredited nursing program.
Graduate of an accredited masters of nursing program.
Meets the requirements determined by the State of Wisconsin or Illinois to perform duties defined as those of a Nurse Practitioner.

Certification and Licensure

Hold valid RN license in state of practice.
Hold valid APNP license in state of practice.
