Care Coordination Manager, UCSF Stanyan Hospital CARE COORDINATION Full Time 87286BR Job Summary Determines department objectives in accordance with overall strategic plan. Erroneous decisions or failure to adhere to objectives results in additional costs and serious delays in overall schedules. The final salary and offer components are subject to additional approvals based on UC policy. Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement. The salary range for this position is $144,200 - $350,000 (Annual Rate). Stanyan serves a diverse patient population, including a significant cohort of older adults, as well as patients and families representing the many languages and cultural backgrounds of San Francisco. This position supports the Care Coordination department staff with developing safe, comprehensive and appropriate disposition plans across the entire Hospital Medicine service and in the Critical Care environments, including the Emergency Department and Intensive Care Unit, as well.
