Equally important, the Director must be a skilled advocate for the Care Management Department, championing its priorities and needs while remaining aligned with the broader goals of the hospital and health system to ensure seamless, patientcentered care.

Summary:

The Director of Care Management effectively provides leadership in the growth, development, and implementation of care management initiatives at Maine Medical Center and serves as liaison to external agencies relative to regulatory compliance and care management. The Director develops and implements a Care Management Plan that supports Maine Medical Center's mission, vision and values. The Director of Care Management is responsible and accountable for developing systems and processes for Care Management across the MaineHealth System. He/she is responsible to plan, direct and supervise all aspects of the program. The Director ensures that the Care Management Plan supports the strategic imperatives, that the facility meets all accreditation requirements and applicable state and federal related regulations, that appropriate information is obtained and utilized to enhance performance, and that education relative to Care Management is ongoing. In addition, the Director of Care Management acts as a liaison with other departments throughout the hospital and MaineHealth System. The Director is responsible for supporting the strategic plans of both MMC and MaineHealth System. The Director provides oversight of the Care Management, Care Coordination, Complex Case Management and Utilization Management programs and works closely with hospital senior leadership, medical staff leadership as well as other system operational functions including but not limited to corporate compliance, medical staff and medical records offices.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Either Registered Nurse with a BSN and Masters Degree in a Health Care Related field or Masters Degreed professional in Health Care.
  2. License/Certifications: CPHQ preferred. Certification in Case Management preferred.
  3. Experience: At least 7 years acute care experience in a complex academic medical center and care management leadership. 5 years of Acute Care Case Management Experience. Minimum 5 years management experience in healthcare or related field required.
  4. Additional Skills/Requirements Required: Background in business/financial planning, vision, strategic planning and targeted outcomes. Demonstrated skills in the planning and development of multidisciplinary clinical programs and systems in both hospital and community based health and social service delivery systems are essential. Demonstrated proficiency with computer and data entry skills with a high degree of accuracy required.
  5. Additional Skills/Requirements Preferred: Experience with InterQual, Milliman, Midas and EPIC preferred. Experience in performance management, care management, systems thinking and application of quality management principles desired. Must possess and utilize negotiating skills to be recognized as a collaborator and team builder.
