Director of Care Management

Telligen is seeking a dynamic and experienced professional to join our team as the Director of Care Management . This pivotal role will focus on directing all activities related to the care and case management of Class Members. The ideal candidate will ensure successful involvement of each Class Members' treatment team, adjusting assignments and partnerships, as needed.
This position requires prior approval from the Healthcare Finance Services (HFS) and is contingent on award of the contract.

What you'll do:
  • Lead and oversee contract(s) and assigned projects, ensuring timely and accurate completion within budget and to client satisfaction. Serve as an operational liaison between clients and Telligen.
  • Manage client expectations and partner with stakeholders, as necessary, for a contract.
  • Stay up to date on healthcare trends.
  • Recruit, develop, and manage a professional team to achieve organizational goals.
  • Ensure accountability through performance metrics and feedback.
  • Prepare and manage budgets, ensuring cost-effective performance.
  • Represent Telligen, inclusive of research and delivery of professional presentations.
  • Direct all activities related to Care and Case Management.
  • Monitor and analyze utilization of Class Members.
  • Ensure continuity of treatment team responsibilities.
  • Ensure productivity and caseload expectations are met.
  • Perform additional duties as assigned.
Required Skills and Experience
  • A four-year degree in healthcare management, nursing, or public health, or equivalent experience in health or human services roles
  • 5+ years of experience in a healthcare environment
  • 5+ years managing professional staff
  • Proven experience in a fast-paced, deadline-driven environment
  • Experience with Medicaid programs
  • Prior approval by HFS (Illinois Department of Healthcare and Family Services) required
  • Must be a Licensed Practitioner of the Healing Arts (LPHA) or otherwise approved by HFS
  • Some contracts may require U.S. Citizenship
  • Regular and predictable attendance required
  • Travel: 15% local and overnight
Preferred Skills and Experience
  • Knowledge of Illinois Medicaid and HFS regulations
