As a Nurse Team Lead - Community Based, you will play a critical leadership role within our Young Adult Intensive Outpatient Services (IOS) program. This position provides both direct clinical care and daytoday leadership support for communitybased nursing staff, ensuring coordinated, safe, and effective care for clients with complex medical and behavioral health needs. You'll serve as a clinical resource, mentor, and liaison across teams while promoting collaboration, accountability, and quality outcomes.


What You'll Do

  • Provide internal and external care coordination to support integrated physical and behavioral health care for clients with complex needs.
  • Deliver direct nursing services including assessment, triage, education, and care coordination to improve client health outcomes and satisfaction.
  • Serve as a lead and support resource for communitybased nursing staff, providing guidance, mentorship, and daily operational support.
  • Act as a liaison with internal teams and external providers to ensure seamless coordination of services and continuity of care.
  • Establish therapeutic relationships with clients to assess needs, provide education, and support treatment goals.
  • Screen clients for health concerns related to psychotropic medications and other medical conditions; provide education and referrals as indicated.
  • Observe, assess, and document client mental status, medical concerns, and progress; communicate effectively with interdisciplinary team members.
  • In collaboration with medical providers, monitor laboratory work, address abnormalities or concerns, and ensure accurate documentation.
  • Triage and respond promptly to client and provider inquiries related to care coordination and medication management.
  • Promote a positive, collaborative team culture and actively participate in teambased models of care.
  • Support operational functions including daily staffing coverage, callout management, and timesheet review in coordination with the Nurse Manager.
  • Serve as a backup to the Nurse Manager when offsite and assist with onboarding and training coordination for new nursing staff.


What You Bring

  • Registered Nurse (RN) with a current, unrestricted Colorado license; BSN preferred.
  • Minimum of two (2) years of related clinical nursing experience with diverse populations in physical and/or behavioral health settings.
  • Demonstrated skill in nursing assessment, including evaluation and interpretation of medical and behavioral health information.
  • Knowledge of nursing care management principles and psychotropic medications.
  • Strong written and verbal communication skills, sound clinical judgment, effective problemsolving, and the ability to manage multiple priorities.
  • Excellent interpersonal skills and the ability to work independently and collaboratively within multidisciplinary teams.
  • Proficiency with electronic health records and PCbased applications including Word and Excel.

Compensation:

90,000 - $101,000 annually


The base salary range represents the low and high end of the AllHealth Network hiring salary range for this position. Actual salaries will vary and may be above or below the range based on various factors including but not limited to experience, education, training, merit, and the ability to embody the AllHealth Network mission and values. The range listed is just one component of AllHealth Networks' total compensation package for employees. Other rewards may include short-term and long-term incentives as well as a generous benefits package detailed below.
