Sr. E Referral Specialist Lead - Hospice

The Senior E-Referral Specialist will serve as team lead responsible for the day-to-day performance of an area, managing referral receipt, workload, and timeliness as well as participating in referral processing. The e-referral team is responsible for receiving and responding to community, physician, patient, and family request for hospice. This position will record and process referral information as well as document in patient information databases and act as the critical liaison between referral sources and our sales and operations teams. The Senior E-Referral Specialist will train, schedule, and mentor junior teammates and assist with management projects that improve the effectiveness and efficiency of the portal team.

* Perform required analysis, decision-making, and data entry accurately and timely.
* Assist in team led and management duties.
* Patient information and forms are managed and forwarded to the appropriate department per procedure.
* Monitor all assigned portals for new referrals, messages, bookings, and updates.
* Work with the intake department and local branches to ensure communication of all booked referrals, referral sources messages and updated documents coming through the portal and to minimize any delays in care of our patients.

* Associate's degree preferred; High School Diploma required.
* 1-2 years experience in related healthcare position.
* Previous EMR portal experience preferred.

* Team lead or management experience preferred.
