Position Summary Monitor clients' health and medication needs. These services include documenting medication lists, overseeing medication distribution, and providing medical case management and reminders for medication adherence. Maintain medication logs, monitor vital signs, organize medication storage, and offer health education. Collaborate with community health providers, internal departments, and social services agencies. Coordinate with pharmacies, assist with healthcare appointments, and ensure accurate, real-time documentation of all services to promote continuity of care. Essential Functions Medication Management
- Document participants' current medications, including prescriber name, medication name, dosage, frequency, duration, and start date.
- Monitor and support clients as they self-administer prescribed and over-the-counter medications.
- Oversee the distribution and storage of medications, ensuring accuracy and compliance
- Maintain accurate and real-time medication logs, including monthly medication lists, expiration dates, and discontinued medication notices.
- Conduct regular audits and accurate counts of all medications; ensure expired or abandoned medications are properly disposed of per policy.
- Coordinate with pharmacies to ensure timely receipt of medications.
- Interface with Facility Monitor to ensure secure collection and transfer of medications, maintaining an unbroken chain of possession and loss prevention protocols.
Medical and Health-Related Services
- Provide basic nursing services such as monitoring vital signs.
- Offer medication management support including reminders, monitoring, and patient education to improve compliance.
- Deliver contingency management support for participants in stimulant use disorder programs.
- Offer health education, resources, and counseling on relevant health topics.
- Develop and facilitate Health and Wellness Education Groups and classes for participants.
Client Support and Case Coordination
- Assist participants in scheduling and accessing primary healthcare services.
- Serve as a liaison between participants and external healthcare providers to ensure continuity of care.
- Provide encouragement and support to help clients maintain consistency with their medication regimens.
- Collaborate and communicate effectively with internal teams and external partners to support participant's well-being.
- Must be able to manage and support the team during crisis situations.
Training and Compliance
- Ensure compliance with all applicable regulations including HIPAA, funder requirements, and TLC policies.
- Maintain bi-annual Certified Nurse's Assistant Certification.
- Review and update internal procedures for medication handling and documentation as needed.
Documentation and Reporting
- Accurately document all services and interactions in a timely manner using appropriate charting formats.
- Conduct periodic reviews to ensure compliance with contract documentation standards
- Communicate regularly with program leadership regarding procedural updates and challenges.
- Maintain a case file for each participant.
General Responsibilities
- Provide trauma-informed, compassionate care to clients who may have experienced high levels of distress or behavioral health challenges.
- Support the Facility Monitor team and perform other related duties as assigned by the Program Manager.
- May require availability for evening, weekend, and holiday shifts as needed.
- While involved in health-related tasks, this position does not include the provision of medical services.
Working Conditions This position requires the ability to walk, stand, bend, squat, climb, kneel, and twist on an intermittent or frequent basis. Must be able to grasp, push, and pull objects such as file drawers, and reach overhead. Requires the ability to operate standard office equipment, including a computer, fax machine, and telephone. Must be able to lift and carry items weighing up to 25 pounds. The role involves primarily site-based work, with occasional office duties such as filing, copying, and attending staff meetings. Minimum Qualifications
- Certified Nursing Assistant (CNA).
- At least 6 months experience in a similar capacity in residential care or hospital care.
- Must be able to work in a stressful environment with multiple competing priorities.
- Must accept risk of exposure to communicable diseases and occasional combativeness from clients.
- Must obtain First Aid/CPR certification and pass health screening within 30 days of hire date.
- Must provide evidence of TB clearance within 7 days of hire, and annually thereafter.
- Must complete and pass DOJ Fingerprint background check.
Skills, Knowledge & Abilities
- Knowledge of and experience working with sub-populations of the homeless, substance abusers, and the mentally ill is preferred.
- Ability to work in a multi-cultural environment.
- Ability to transcribe medication name and dosage information from prescriptions and/or medication containers to facilitate accurate documentation.
- Must have strong written and verbal communication skills appropriate for responsibilities.
- Ability to work as part of a multi-faceted team.
- Skilled with Microsoft Word, Excel, & Outlook.
- Must have good audio and visual perception to assist residents in emergency situations.
