Position Description:

* Conduct interdisciplinary rounds with physicians, mental health staff, and correctional healthcare teams to discuss placement options, discharge planning, release timelines, and continuity of care needs.

* Review custody and medical records to identify inmates eligible for CalAIM services and upcoming releases.

* Collaborate with custody, healthcare, and security staff to maintain accurate release timelines and ensure timely coordination of care transitions.

* Coordinate discharge planning by involving patients in care decisions, identifying support systems, arranging follow-up care, and providing education on medication management and community resources.

* Facilitate referrals and coordination with community agencies, shelters, employment resources, substance use counselors, and social service organizations to support successful reentry and continuity of care.

* Assist patients with completion of clinical and financial documentation required for community-based services and treatment programs.

* Organize medical and transition records and provide guidance regarding transportation options, placement resources, and community assistance programs.

* Serve as liaison for inpatient care management, involuntary examinations, and crisis stabilization referrals to ensure appropriate placement and continuity of behavioral health services.
