Job Description
The Referral Care Coordinator is responsible for ensuring referrals are accurate, complete, and actively managed from assignment through completion. This role supports timely access to care by validating referral information, coordinating referral documentation, tracking referral status, and facilitating communication between providers, clinics, and patients as needed. The Referral Care Coordinator plays a key role in maintaining closed-loop referral management, ensuring referrals progress through the system without delays or gaps. Complex, urgent, or high-risk referrals are escalated to the Referral Navigator for advanced coordination. What You'll Be Doing Referral Coordination and Documentation

Closed-Loop Referral Management
Supports the Referral Navigator as requested to ensure continuity of care across the care continuum. Builds positive relationships with patients, providers and hospital staff with a focus on customer service and quality care. Ensures a culture of exemplary customer service and responsive patient centered care. Functions as a member of the healthcare team to ensure compassionate and coordinated care based on respect for patient's preferences, values, and needs. Performs other duties as assigned. Adapts to change, accepts new assignments, and improves ways of doing things with grace and enthusiasm. Promotes quality improvement, staff and patient safety, and cultural diversity through department operations and by personal performance. Provides service in a manner that is appropriate for the patient's age; demonstrates knowledge and skills necessary to meet the patient's physical, psychosocial, educational, and safety needs. Presents a courteous and helpful demeanor, appropriate for age, to all patients, visitors, other UH employees/medical staff members, or any other person an employee encounters while representing University Health. Maintains current knowledge related to applicable statutes, regulations, guidelines and standards necessary to perform job duties in accordance with the requirements of the Corporate Compliance Plan. Complies with the requirements of the Code of Conduct, Corporate Compliance Plan and Compliance Policies and Procedures, including training requirements.
