The Population Health Improvement Assistant will perform administrative duties and documentation in support the completion of Population Health, Value Based Care, and Quality objectives. This will include patient outreach and engagement, pre-visit planning work, data tracking, or other coordination or administrative processes needed to meet value-based requirements. Additional Information: Additional Information: This position supports the clinics administrative and clinical operations through: panel management, data-collection, documentation for quality work, Care Gap Closure, and supports the Population Health Improvement Manager (PHIM) set or implement standard work and best practices.
